This is not a vague “lose weight, feel better” claim; it has a specific mechanism and a dose-response relationship that holds up across studies.

Why fat tissue converts testosterone into estrogen

Adipose tissue is hormonally active, and it expresses an enzyme called aromatase that converts testosterone into estradiol, a form of estrogen. A 2025 study in the Journal of Clinical Endocrinology & Metabolism found aromatase expression significantly elevated in adipose tissue from men with obesity compared to lean men, concluding that “increased aromatase activity in adipose tissue contributes to lower testosterone and hyperestrogenemia in men with obesity” (Ahmed et al., 2025). The famous data point, that a week of 5-hour nights drops testosterone by 10-15% in young healthy men, comes from a small but rigorous study in JAMA (Leproult & Van Cauter, 2011). It is real and widely cited, and it is also one study on 10 subjects.

What the newer research actually shows

A 2021 systematic review and meta-analysis pooling multiple studies found a more nuanced picture. Total sleep deprivation, staying awake 24 hours or more, produced a clear, significant drop in testosterone (standardized mean difference -0.64). Lever 4: training barely moves the needle directly

This is the one that will annoy people, including a version of me before I read this research. Resistance training is usually presented as the headline lever for testosterone after 40: lift heavy, hit compound movements, and your hormones follow. The direct evidence does not support that claim as strongly as the fitness industry assumes.

A 2019 systematic review and meta-analysis in Frontiers in Physiology examined short-term exercise training and basal (resting, not immediately post-workout) testosterone in older men. Resistance training produced essentially no change in basal testosterone (standardized mean difference -0.003, p = 0.986), leading the authors to conclude that “resistance training does not significantly influence basal testosterone in older men” (Hayes & Elliott, 2019). The same review found endurance training and HIIT showed small but positive effects, the opposite of what most lifting-focused content would predict.

Why lifting still belongs in the plan

The same review noted that many resistance training studies still produced meaningful strength and muscle gains despite no measurable rise in basal testosterone, suggesting elevated testosterone is not mechanistically necessary for those adaptations. My own numbers, for what they are worth

I do have a dedicated hormone panel. Total testosterone came back at 17.12 nmol/L (494 ng/dL), squarely mid-range against a lab reference of 8.2-32.2 nmol/L. Free testosterone was 357 pmol/L against a range of 160-560 pmol/L, also mid-range, and SHBG and albumin both landed mid-range too.

Nothing dramatic, nothing a clinic would flag, and nothing that needed a prescription to fix. The doctor’s note was “everything looks normal,” which taught me something the number alone does not: normal on a lab report and how you want to feel and perform are not always the same thing, and a mid-range result is not, by itself, a reason to reach for TRT.
